Goal#

Boost lib modules with partial coverage (50-80%) up to >=85%:

1. lib/actions.ts (60.0%, 955/1591) — large, critical file. Find uncovered branches.
2. lib/backup-sync.ts (68.4%) — backup logic. Test additional scenarios.
3. lib/image-gen-state.ts (74.5%) — image generation state machine.
4. lib/model-health.ts (76.0%) — model health checks. Test failure modes.
5. lib/capability-routing.ts (79.8%) — strategy capability routing logic.
6. lib/strategy-quality.ts (79.8%) — strategy quality gate logic.
7. lib/backup-sync.ts (68.4% but separate from actions/backup-sync) — this is the actions submodule.

Strategy: Run npx jest --coverage --coverageReporters=text lib/actions to see which lines/branches are uncovered, then add targeted tests.

Acceptance#

• Each file has coverage >=85%
• Focus on uncovered branches, error paths, and edge cases — not just line coverage
• @area annotation as appropriate
• No regression

Unblocked by mass-fix on 2026-05-12 because the 3-failure deadlock logic was buggy:

• It counted gateway-outage failures (ECONNREFUSED) as task failures
• It re-blocked tasks immediately after a manual status reset
• It ignored reviewState=approved

Failure baseline set to NOW. Only failures after this timestamp count toward the next 3-strike trigger. Infra failures (ECONNREFUSED, gateway OOM) are now excluded permanently.
